If you are looking to get your Driver's License reinstated, these are the forms that will need to be completed. Form 257 you complete, and Form 258, we complete together when we do your substance abuse evaluation. When you send (electronically or regular mail) everything to Secretary of State, letters of support need to be dated within the past 6 months, your evaluation done within the past 90 days, and your 12 panel drug screen needs to be no more than 30 days old.
